🌿 Photonics-Driven Regeneration: Reflections from Clinical Practice

✨ A Note from the Conference

I recently attended the Spring Scientific Meeting of the Laser and Photonics Medicine Society, where I also had the honor of serving as a session chair.The conference brought together diverse perspectives to explore an important theme: “photonics-driven regeneration and repair.”

🔬 Exosomes: Promising, Yet Still Under Investigation

One of the most discussed topics at the meeting was exosomes.Their applications now extend across skin regeneration, hair restoration, hair graying, vitiligo repigmentation, and post-laser recovery.

However, from a scientific standpoint, I believe it is important to remain cautious.The biggest challenge lies in their heterogeneity.

Variations in source (animal, plant, or human) and differences in isolation methods can significantly affect their composition and outcomes.We still do not fully understand which components are truly responsible for their effects.

🧬 Exosomes in Vitiligo: Hopeful but Not Yet Proven

This is one of the most common questions I receive:“Are exosomes effective for vitiligo?”

Based on current evidence, this application is still in an early exploratory stage.Most data come from case reports, and robust large-scale clinical trials are still lacking.

Vitiligo itself is a highly complex condition involving immune dysregulation, melanocyte biology, and regenerative processes.

Even within the same patient, responses may vary across different body areas.This is why every new therapy requires careful validation.

💉 On Delayed Inflammatory Reactions (“Overfilling”)

Another topic that stood out was delayed inflammatory reactions following injectable treatments—sometimes referred to as “overfilling.”

These reactions may occur after hyaluronic acid fillers or biostimulatory agents, and although uncommon, they are closely related to immune responses.

They involve interactions between macrophages, fibroblasts, and inflammatory pathways, requiring thoughtful management.

🌱 The Essence of Medicine: Navigating Uncertainty

To me, medicine is not only about adopting new technologies.

It is about making sound decisions based on scientific evidence—even in the presence of uncertainty.

And ultimately, guiding patients toward a safer, more sustainable path forward.
